Outline of Annual Research Achievements

1) 症例収集と疫学調査：前年度に続いて、口腔癌および粘膜下線維症を含む前癌病変とくに上皮内癌症例収集について、下記のミャンマーの調査地域基幹病院の研究協力者Myint博士らの協力のもとに抽出調査と病理組織標本の検討を実施した。
2) バーチャルスライドデータベース閲覧システム：前年度で構築したバーチャルスライド閲覧システムを用いて、収集した症例の病理組織・細胞標本を海外の研究協力者と国内研究分担者らが遠隔的に症例を閲覧して、診断を確認参照した。
3) ミャンマーにおけるコホート調査結果についての検討：ミャンマーの調査地域タングーで実施した噛みタバコ常習者・非常習者からなる対象住民について実施した調査のうち、血液試料における抗酸化物質レベルの測定結果と細胞診ならびに組織生検標本診断結果について取りまとめるため、研究代表者朔が現地に出張し、研究協力者Myint博士らと共同で検討して、報文の取りまとめを行った。Myint博士らが来日する予定であったが、今年度中は調整がつかず、来年度とすることになった。
4) 口腔粘膜扁平上皮癌・上皮内癌に関する分子病理学的実験：前項で収集した症例ならびに本学で収集した標本のうち、上皮内癌を中心に、口腔癌細胞の特異的分子発現を角化と細胞接合に関する解析を完成した。側方浸潤における界面形成部において細胞競合現象が生じるという仮説を証明するためのプロテオーム解析実験を行ってきたが、界面特異的な分子発現を確認した。一方、現地で収集した組織切片より核酸を抽出してPCR法による遺伝子断片増幅を試みたが、収率が低いため、本解析計画については、次年度に引き続き検討することとした。細胞診については、従来の核異型性を基準にしたベセスダ方式では困難な分化型口腔癌の診断に新たな細胞質所見を導入することの有用性を確認した。

Reason

基本的に当初の「研究の目的」に沿って調査研究を実施してきた。現地研究協力者側の勤務機関異動のため調査および解析がやや遅れている。現地調査の結果の一部を解析して報文作成を開始した。口腔癌細胞の特異性に関しては、角化ならびに細胞接合に関して論文を完成した。口腔癌側方浸潤界面における発現分子の解析についても概要をまとめた。細胞診標本の解析から新たな口腔細胞診の診断基準を提案した。前項ならびに後項にも記載するとおり、核酸抽出に困難があったため、遺伝子解析実験がやや遅れている。

Strategy for Future Research Activity

海外調査では現地共同研究者の協力が重要である。今年度からミャンマー国政府による政府機関の機構改革の一環として、ミャンマー側の研究協力者医学研究所Myint所長は伝統医学研究所の所長に配置換えとなって多忙を極めている。また前任の医学研究所の機構改革によって旧職員の配置換えもあり、今年度の研究については、相手国機関側に柔軟性をもって対応していく必要があるが、現地での組織切片の核酸の良好な保存法を検討すると共に、日本側でも核酸抽出技術の改善にも努め、遺伝子レベルの実験を進めていく計画である。

Causes of Carryover

現地研究協力者のMyint博士らに日本への招聘を要請していたが、今年度内の調整がつかず、来年度に実施することになった。現地で収集した組織切片よりDNAを抽出してPCR法による遺伝子断片の増幅を試みたが、収率が低いため、DNA酸化傷害関連分子の発現プロフィールの解析に供する試料が準備できなかった。以上の旅費ならびに解析費用を次年度に繰り越すことになった。

上記のとおり、現地研究協力者らの招聘ならびにDNA実験費用に充当する予定である。
