Neural mechanism of the effect of neurorehabilitation for pain

Outline of Annual Research Achievements

橈骨遠位端骨折は、他の骨折より複合性局所疼痛症候群(CRPS)の発症率が高い。今回、慢性化を予防する目的で、我々が開発した腱振動刺激に基づく運動錯覚療法の効果の検証を疼痛の緩和のみならず、運動障害にも効果を示すか明らかにした。また、その際の脳活動を記録し、脳波分析から効果のメカニズムを検証した。対象患者は手術を施行した橈骨遠位端骨折術後患者22名である。手術様式は全て掌側プレート固定であった。この患者を運動錯覚群11名、コントロール群11名に分けた。主アウトカムには、痛みによる手の運動機能障害を調べるThe Patient-Rated Wrist Evaluation and Pain(PRWE)を用いた。介入は手術後7日間行った。昨年度の成果同様に運動錯覚群において安静時痛、運動時痛および痛みの情動的側面を示すPain Catastrophizing Scale(PCS)のスコアの反芻の項目で有意な減少を認め、強い痛みの緩和効果が示された。またコントロール群と比較してその効果量も大きかった。PRWEのスコアに関しても、運動錯覚群で有意な改善を認めた。これらの結果から、腱振動刺激による運動錯覚が、痛みの緩和のみならず運動機能障害にもポジティブな効果を示すことが明らかになった。加えて、運動錯覚惹起の際の脳活動を脳波計(32ch)を用いて記録したところ、運動錯覚時には運動関連領域のHigh α波が減衰すること、すなわち活動が増加することが明らかになった。また、C3、C4(一次運動野領域)の活性化とVisual Analogue Scale(VAS)を用いた安静時疼痛の値に有意な負の相関を認めた。すなわち、活動が高いほど痛みを緩和させることが明確になった。この結果は、運動錯覚によって運動関連領域の活性化し、それに基づき下行性疼痛抑制のメカニズムが作動したことが示唆された。
