近年、糖尿病が増加し、網膜症や腎症、動脈硬化症などの合併症が社会問題となっている。糖尿病の治療には食事療法、運動療法、薬物療法があるが、これら既存の治療では高血糖や合併症治療として効果不十分な症例も多く、新規の治療法が求められている。脳由来神経栄養因子Brain-derived neurotrophic factor (BDNF)は血糖低下作用を有する事から新規糖尿病薬として期待されるが、糖エネルギー代謝への作用や糖尿病合併症への関与など不明な点が多い。 これまでに我々はBDNFを7日間連日皮下投与した正常マウスでは、有意にえさの摂取量と空腹時血糖が低下することを報告した。またBDNFの受容体であるtrkBが膵島α細胞に発現しており、BDNF添加により単離膵島からのグルカゴン分泌が抑制された。またBDNFノックアウトマウスでは血糖値がwildtypeに比べ有意に高値だった。これらの結果より、BDNF は食欲抑制作用に加え、膵α細胞からのグ ルカゴン分泌を抑制することにより血糖を低下させる可能性があることが示唆される。 一方BDNFは血中ではそのほとんどが血小板のα顆粒内に存在し、心筋梗塞など血管障害では保護的に作用することが報告されている。今回、共同研究者の古川和郎らは、慢性的な高血糖状態で蓄積する最終糖化産物(AGE)がヒト血小板においてSrc family kinase(SFK)の活性を介してBDNFの放出を促進し、血小板内BDNFの枯渇をもたらしている可能性を報告した(Cardiovasc Diabetol. 2017;16(1):20. doi: 10.1186/s12933-017-0505-y)。糖尿病患者ではAGEにより血小板内のBDNFが枯渇し、BDNFの血管保護効果の減弱により血管合併症が促進される可能性が示唆された。
