本研究は同種造血幹細胞移植成績向上を目指し、次世代シーケンサーによる網羅的なHLAならびにKIR領域のリシーケンス法を確立することを目的とした。34のHLA遺伝子、17のKIR遺伝子の全長をカバーするオリゴプローブをカスタムデザインし、シーケンスキャプチャー法による網羅的な解析手法を確立した。また、得られたデータ解析手法として、1) IMGT/HLAデータベースに基づくHLAタイピング、2)コピー数多型および構造多型検出によるHLAハプロタイプ構造解析、3)網羅的一塩基置換(SNV)および挿入欠失(Indel)検出、の異なる3種類の手法による解析法を確立した。さい帯血移植ペア882検体のシーケンス解析の結果、全ての検体において97.2%以上のシーケンスリードがヒトゲノム標準配列にアライメントされ、その平均リードdepthは84.5x、平均して94.5%の領域が少なくとも10リード以上でカバーされていた。HLA領域における検出された1検体あたりの平均SNVならびにIndel数は4720.6ならびに261.2ヶ所であった。全検体では17,575のSNVと1,322のIndelが認められ、これらのSNVの81.7%、Indelの89.6%は遺伝子領域、さらにこのうちのそれぞれ10.2%および3.4%がエクソン領域の多型・変異であり、SNVの69.9%がアミノ酸の違いを伴うミスセンスおよびナンセンス変異であった。本シーケンスから得られた多くの多型変異のうち、多数のアミノ酸置換を伴うSNVが認められたが、これらのアミノ酸置換のうち、184ヶ所はdbSNPデータベースに登録の無い、新規のSNVであった。これら新規のSNVも含め、同種造血幹細胞移植成績との関連を精査した。
