Evaluaion of Human Cytochrome P450 3A Enzymes in Hepatic and Placental Cells by Thalidomide and Relevance to Bioactivation

Outline of Annual Research Achievements

サリドマイドのヒトにおける代謝的活性化には、活性化酵素であるヒトチトクロムP450 3A酵素の自己誘導と、P450 3Aの協同性を示す芳香環酸化反応が重要であることを提示した。催奇形性を回避して開発された次世代サリドマイドのアミノ化合物であるレナリドマイドはP450 3A誘導効果があるが、酸化的代謝は受けないこと、一方、ポマリドマイドはP450 3A酵素誘導はないが芳香環酸化を受けることも明かにしていた。ヒト肝移植マウスにサリドマイドを経口投与し、その薬物血中濃度情報を用いる簡素な生理学的薬物動態(PBPK)モデル手法でヒト濃度推移を予測した。ヒト肝キメラマウスでのサリドマイド血中濃度実測値をもとに動物PBPKモデルを構築し、多様な薬物動態パラメータのうち、生理学的数値をヒト化し、経口暴露時の代謝物を含めたこれらのヒトPBPKモデルを構築した。サリドマイドではヒト不均衡性代謝物となる芳香環5-水酸化サリドマイドの生成がヒト肝移植マウスの血中に特徴的に見出された。親化合物のヒト肝移植マウスの実測血中濃度から、ヒトの仮想投与血中濃度曲線を作成すると、報告されている薬物血中濃度と一致したことから、代謝物のヒト曝露が推定された。今後、次世代サリドマイド関連薬物のげっ歯類とヒトとの薬物血中動態の乖離をヒト肝細胞マウスあるいヒト胎盤培養細胞を用いてさらなる解析を実施し、ヒト不均衡性代謝物となる芳香環5-水酸化反応進行の程度が安全な次世代サリドマイド関連化合物開発の基盤的指標となることが期待される。
