Outline of Annual Research Achievements

食後、特に脂肪負荷後、血中の中性脂肪(TG)が著明に上昇することはよく知られている。そのため食後に血中に脂質を速やかに分解するための酵素、リポ蛋白リパーゼ（LPL）が増加すると言われてきた。しかしながら、我々はヘパリン非投与血清を用いて測定した血清LPL濃度が、ヘパリン投与後のLPL濃度よりも生理的意義が高い可能性を報告してきた。従って、ヘパリン非投与下で、脂肪負荷、糖負荷をして負荷後血中LPLが増加しないことを証明した (Ishiyama N,et al. Clinica Chimica Acta 2017; 464: 204)
その結果として、LPL濃度が上昇しないことが、食後レムナント・リポ蛋白が著明に増加する可能性を次の論文にて報告した。
つまり、食後に血中TGが増加する場合、どのリポ蛋白分画のTG が脂肪負荷等の後にどの程度増加するのか、明らかとなっていなかった。本論文ではLPLが食後に増加しない結果として、レムナント分画中のTG含量が、健常人において食後4時間前後で著明に増加し、その占める割合は脂肪負荷の場合は90%近くになることが明らかとなった。また冠動脈硬化症等においては、レムナントTGの増加がより著明になり、もっとも上昇する時間が６時間後と遅滞し、かつクリアランスが低下することが明らかとなった。その理由は、レムナントの肝臓でのレセプターに対するリガンドであるLPL濃度が低いことに関係する可能性を示唆した(Nakajima K,et al.
