高齢者連続432症例を検討したところ、入院時常時薬剤数は平均5.1剤であった。ポリファーマシー患者は52.3％にのぼり、ステロイドなどの免疫抑制治療を行わない群では、薬剤数が入院時より削減されていた。削減薬剤の多くは、ビタミン類、胃粘膜保護薬を含む胃潰瘍治療薬、カルシウム拮抗薬などであった。削減理由は、無効薬の漫然投与の中止、重複薬の整理、過度の降圧などの有害事象のためであった。退院時には平均1.1剤の薬剤が削減されていた。これまでの定説では、急性期病院に入院すると、薬剤数が増加するといわれてきたが、多職種で処方をレビューすることにより、ポリファーマシーの高齢者の20％が入院を契機に非ポリファーマシー状態へ移行することがわかった。急性期病院への入院がむしろ、高齢者のポリファーマシーを見直すきっかけとなっていると考えた。
