The effect of HPV status on radiation sensitivity of photon and carbon-ion radiotherapy in head and neck squamous cell carcinoma

頭頸部扁平上皮癌はHPV感染に起因した腫瘍であるかどうかで、放射線感受性が異なることが報告されている。臨床的にはHPV negative扁平上皮癌はpositiveな扁平上皮癌に比べて、X線抵抗性であり生存率も不良である。当施設では放射線抵抗性腫瘍である頭頸部非扁平上皮癌の重粒子線治療を施行しており、良好な局所制御を認めており、HPV negative扁平上皮癌に対しても有効である可能性がある。しかし、HPV statusによる重粒子線治療、X線治療の効果については、基礎的な研究はなされておらず、未だ不明である。
そこで今研究では、in vitro研究や臨床検体を用いたTranslational researchを行い、HPV statusの有無による重粒子線とX線の照射効果の差について検討した。
まず、10種類にわたる頭頸部扁平上皮癌細胞株を用い、HPV statusやその他の遺伝子変異や放射線抵抗性蛋白を確認した。In vitroにおいて、Clonogenic survival assayやFlow cytometryを用い、重粒子線・X線における放射線感受性を確認した。
また、当院において頭頸部扁平上皮癌に対して化学放射線治療を施行した患者群をリストアップし、予後調査（局所制御率、全生存率、無病生存率など）を行なった。HPV statusについては、腫瘍の生検検体のp16の免疫染色の結果をサロジェートマーカーとして評価し、治療成績（特に放射線治療抵抗性）との比較を行った。これらの結果については、今後学会発表、論文発表予定である。
