Outline of Annual Research Achievements

緑内障における視野進行には眼圧が寄与しているため、臨床現場では眼圧下降治療を行っている。しかし、この治療後において、実際の患者で視野進行と眼圧との関連がどうであるのかを、多施設データを用いて調べた研究は過去になかった。そこで国内10施設から10回以上の視野記録のある805例1348眼のデータを集め、視野進行に寄与している因子を調べた。この結果、平均眼圧は視野進行に関連していないものの、眼圧変動が大きいほど進行が速いことが分かり、報告した。更に視野データに眼圧を組み込むアルゴリズムを作成し、同多施設データを用いてこのアルゴリズムによる視野進行の予測精度を調べた。この結果これまでのように視野感度を時間に対して線形回帰するよりも有意に予測精度が良いことが確認され、報告を行った。また、緑内障性視野進行には様々な因子が関連しているが、同データを詳細に解析した結果、喫煙習慣があると、通常の緑内障とは逆に下方視野の視野障害が促進されることが確認された。また、深層学習法（ディープラーニング）を用いて視野を解析するアルゴリズムを構築し、この方法を用いることで、これまでのいかなる方法よりも早期に緑内障性視野異常を検出できることを報告した。また、光干渉断層計の計測結果を機械学習法の一種であるランダムフォレスト法で解釈することで早期の緑内障診断をこれまでのどの方法よりも早期に正しく行えることを報告した。更には角膜のヒステレシスが緑内障の進行に深く関連している因子であることを特定し、報告した。他にも、緑内障患者の生活不自由度を推測するitem bankを構築し、これによる結果をラッシュ法で解釈することで、正確な推定が行えることを発見し、報告した。
