Outline of Annual Research Achievements

健康を障害され入院している患者とその家族は、非日常的な医療に関する意思決定を求められる。患者とその家族の意思決定には医療者との関係が関連しているが、病名の告知の有無によって及ぼす要因の異なることが、和文献から示唆された。そして、家族は、病名の未告知では、未告知による揺らぎ、未告知による後悔、未告知の選択の納得が患者のために意思決定する効力に関連していた。病名の告知では、患者の意思がわからないことによる困難感、患者の思いを知ったことによる困惑、患者本人の意思とは異なる決断、死別後の後悔や葛藤、精一杯のことをしたとの思いが指摘されていた。
今年度、研究協力者であるDr. Marie T. Nolan（School of Nursing, Johns Hopkins University）とともに、研究の現状や今後の研究の進め方について、意見や情報交換を通して、医療の意思決定プロセスにおける判断能力のある患者の家族の包含度に対する意向と家族の意思決定の効力感とその支援について、示唆を得た。
研究代表者の所属する機関の倫理委員会において承認後、2施設における倫理委員会での承認あるいは承諾を得て、実施し分析中である。
また、臨床看護師との事例検討会では、特に高齢者の退院支援において、患者と家族の合意形成における力、退院後のケアの調整力や療養場所の移行準備の視点から看護介入を行う点での討議を行った。
