分子標的治療薬に対する二次遺伝子変異は予防可能か

Outline of Annual Research Achievements

平成２７年度は以下の研究１を次世代シークエンサー（NGS）とmRNAアレイ解析を用い、研究２をConfocal Microscopeを用いて行った。
１．GISTイマチニブ耐性細胞の遺伝子解析：GIST細胞株由来の耐性株（二次遺伝子変異有り２株、無し２株）と親株を用い、NGS解析並びにアレイ解析と299 候補SNV に対してUltra-deep sequencingを行った。結果、①．親株に比しイマチニブ治療開始後には遺伝子変異は殆ど起こっておらず、逆にタンパク発現はトランスクリプトームで見ると大きな変化を示した。一方時間が経過した、二次遺伝子変異有り・無し両者とも耐性株ではSNVやCANに大きな変化が起こっており、その変化は培養時間依存性に高くなっていた。しかし、耐性株のトランスクリプトームの変化は殆ど認めなかった。②．親株のUltra-deep sequencingを行ったがイマチニブ耐性後に確認されたKIT二次遺伝子変異は、親株には確認できなかった。③．臨床検体と細胞株で、イマチニブ治療前に耐性を起こす共通の変異は認めなかった（論文投稿中）。
２．KIT変異GISTの変異KIT活性化メカニズムの解析：KIT・PDGFRA変異を持つGISTの変異KIT・PDGFRAチロシンキナーゼの活性化部位を、wild type KIT・PDGFRAと比較し検討した。wild type KIT・PDGFRAは細胞膜で活性化しているが、変異KIT・PDGFRAはゴルジで活性化しており、二次遺伝子変異を持ったイマチニブ耐性KITもトランスゴルジで活性化していた（論文投稿中）。
以上より、耐性細胞が治療前に存在する確率は低く、変異KITと耐性株も含め、正常KITとは異なりトランスゴルジで活性化し、増殖シグナルを出していることが明らかとなった。
