卓上型次世代シーケンサーを用いた AMLの迅速かつ安価な予後予測システムの開発

Outline of Annual Research Achievements

卓上型次世代シーケンサー(MiSeq, Ilumina)を用いて迅速かつ安価な予後予測システムの開発をめざし、白血病パネルの確立を試みた。
方法:急性骨髄性白血病(AML)の初発、寛解、再発検体よりDNAを常法通り抽出し、以後の検討に用いた。白血病パネルはAgilent社のHaloplex カスタマイズドパネルを用い、約150遺伝子を選定した。実際の解析は、卓上型次世代シーケンサー(MiSeq, Ilumina)を用い、現在8例のAML検体で検討が終了した。
症例1)非Down症のFAB-M7の初診時2歳の女児、OTT-MALやNUP98-NSD1, CBFA2T3-GLISなどのキメラ遺伝子は陰性だった。本解析で初発時と再発時検体からIKZF1 p.F154Y、KRAS p.G12A、GATA1 p.R270Wが検出された。症例2)初診時7歳男児、本例からはKRAS p.G12DがAllele frequency (AF) 0.46、p.G12AがAF 0.053みつかった。症例3)15歳男児 FAB-M2, 初診時、染色体異常inv(16)(p13.1q22)とFLT3-ITDを低率(AF 0.035)に認めた。本方法で、KIT p.D816Vが初診時検体で検出された。症例4)非Down症のFAB-M7の寛解導入不能な1歳女児例、OTT-MAL, NUP98-NSD1, CBFA2T3-GLISなどのキメラ遺伝子は陰性、本方法で転写因子のPU.1 p.G156Rが検出された。
残りの4例では初発と寛解期の検討で、point mutationは見つからなかったが、SH2B3とSMARCB2にlarge deletionがみつかり、現在解析中である。
考案:このパネルの利点はわずか225ngのDNAから解析可能、実際の解析は1-1.5日で終了、専用解析ソフトを用いるため、bioinfomaticsに詳しくない研究者でもデータ解析可能な点にある。1検体の解析費用は3万円程度と安く抑えられる点は有益であった。今後、AML診断後早期に、予後予測を可能とし、実臨床への応用も可能ではないかと考えられた。
