| Outline of Annual Research Achievements |
マイクロキャビティアレイ液体生検技術を用いて、肝細胞がん（肝がん）循環腫瘍細胞（CTC : circulating tumor cells）を特異的に検出する手法を確立し、治療後の患者を対象とした再発リスク診断法を開発した。CTC検出系の確立はin vitro実験系および臨床研究によって実施した。
（１）肝がんCTC検出系の確立： 肝がん患者の末梢血液中を循環しているCTCを検出する実験系を確立するために、マイクロキャビティアレイの感度・特異性について検討した。（a）血球の通過性、（b）肝がん細胞株HepG2、Huh7、PLC/PRF/5を用いた検出系の確立（in vitro）において、肝がん細胞の特異的な検出率（回収率）はそれぞれ66％、76％、99％であった。さらに、肝がん細胞の定量性に関しては、腫瘍関連マーカーAFP、GPC3、EpCAM、ASGR遺伝子の発現量（mRNA）をreal-time RT-PCR技術にて確認した。
（２）肝がん患者からのCTC検出；臨床研究： （１）で確立した検出系を用いて、肝癌患者19例、肝硬変患者(非担癌患者)11例、健常人7例の全血3mLより検出した。肝癌患者において８９％（17/19）でCTCを検出した。進行度別のCTC数では血清AFP値（<4 vs. 4< ng/mL）：3.9 vs. 97.3個（P < 0.05）、腫瘍個数（1 vs. 2< 個）：13.0 vs. 79.3個、最大腫瘍径（<30 vs 30< mm）：61.9 vs. 59.8個であり、進行と共にCTC数は増加傾向を示すことが分かった。
