Effects of end-of-life discussions on the mental health of bereaved family members and quality of patient death and care

Outline of Annual Research Achievements

本研究は、本邦の終末期がん患者における「終末期の治療の目標や過ごし方に関する話し合い（End-of-Life (EOL) discussion）」が行われた頻度およびその内容を明らかにし、EOL discussionの有無と患者Quality of Life (QOL)、遺族の抑うつ・複雑性悲嘆との関連を探索すること、を目的とした、遺族に対する調査票を用いたサーベイランス研究である。
平成26年度の実績：系統的文献レビューをもとに調査票を作成した。参加183施設において2014年1月31日以前に死亡したがん患者を各施設10名づつ連続に同定し、対象者リストを作成した。各施設より対象者へ調査票を郵送し、回答された9123件の調査票を回収した。回収したデータの入力を完了した。
平成27年度の実績：データクリーニング・固定を完了し、データ解析を行った。解析結果としては、対象となったがん患者の約80%に対してEOL discussionの場が持たれ、EOL discussionあり遺族の方がなしの遺族と比較してうつ・複雑性悲嘆の合併が有意に低いことが示された。また、EOL discussionありの遺族では、遺族評価による「患者の望ましい死の達成」や「終末期に受けたケアの質」の評価も、なしの遺族と比較して、有意に高いことが示された。
平成28年度の実績：解析結果を、国際学会（Palliative Care in Oncology Symposium. Sep 2016. San Francisco, CA, USA）で発表を行った。また、査読ありの国際学術誌へ論文投稿を行い、掲載受理となった（現在印刷中）。
