レビー小体型認知症におけるレム睡眠行動障害の診断・治療法の応用

Outline of Annual Research Achievements

レビー小帯型認知症(DLB)は、しばしば、他の認知症性疾患と誤診されている。診断精度を高め、見逃しを少なくするためには、DLBの臨床診断基準の示唆的特徴にも注目することが重要である。その中でも“①レム睡眠行動障害(RBD)”と“②SPECT/PETで示された大脳基底核でのドパミントランスポーターの取り込み低下” に注目した。
今回、我々は、核医学検査である「脳血流SPECT」、「MIBGシンチ」、「DAT-SPECT」の３つのモダリティを組み合わせることにより診断感度があがるか否かを調査した。対象は、probable DLBと診断された34例（75.0 ± 8.3歳, 男14: 女20）。MIBGシンチとDAT-SPECTはどちらも79%という高い診断感度を有していたが、脳血流SPECTにおいては50%に留まった。
また、３つのモダリティのうち、いずれか１つが陽性とした場合、感度は100%であったことより、これらのモダリティの組み合わせは、DLBの診断感度上昇に大きく貢献することが明らかとなった。
また、サブ解析から、RBDを伴うDLB患者の各モダリティの陽性率は、脳血流SPECTが58.6%、MIBGシンチが100%、DAT-SPECTが96.6%であり、RBDを伴うDLBの診断には、MIBGシンチ、ほぼ同程度にDAT-SPECTの有用性が示唆された。
