Outline of Final Research Achievements

Individual difference with regard to extent of ghrelin expression in gastric mucosa exists among morbid obesity patients. The high Ghrelin expression group exhibited better excess body weight reduction than the low Ghrelin group. Regardless of the ghrelin expression level, all obesity patients with type 2 diabetes mellitus (T2DM) showed the remission for T2DM after laparoscopic sleeve gastrectomy (LSG).
Our study also revealed the re-enhanced ghrelin expression at the lesser curvature of remnant stomach for several cases who underwent sleeve gastrectomy. Serum insulin and c-peptide level of those cases tended to underreach the normal level. In addition, all of patients with re-enhanced ghrelin expression demonstrated rebound of body weight.
Our conclusion and implication is that ghrelin expression level at the stomach is an indirect factor and/or one of the direct factors which affect the improvement mechanism of T2DM resolution after LSG, although it is not the solely direct factor.
