Outline of Annual Research Achievements |
This study was undertaken to demonstrate the importance of the versatile evaluation of articulatory function in cleft lip and palate (CLP) from central and peripheral perspectives. Subjects with CLP receive many surgical interventions to acquire normal structure, however as a result of all these procedures they suffer from orofacial dysfunctions, that cause altered orofacial movements, moreoever sensorimotor integration in the brain during speech articulation. During the 2014 fiscal year, six postsurgical CLP patients [two unilateral CLP (UCLP), four bilateral CLP (BCLP)] and six non-CLP participants took part in fMRI study during covert (C) and overt (O) articulation,. Two post-surgical adult males with CLP [with unilateral CLP (UCLP) and bilateral CLP (BCLP)] and two healthy volunteers participated in 3D MRI movie study. For both MRI acquisitions, a same participant with BCLP showed compensatory misarticulation (CMA). All subjects were right-handed according to the Edinburgh handedness inventory (Oldfield, 1971), non-syndromic, and neither had neurological problems.

Reason
In the C-task, there were no significant differences in the activation patterns between CLP and non-CLP. However, in the O-task, participants with UCLP and BCLP without CMA resembled the activation patterns in non-CLP subjects. Articulatory movements in the midsagittal, horizontal and coronal planes were observed in a volumetric image of the 3-D MRI movie. This technique permite to navigate through the vocal tract, and the possibility to cut the image in different axes and levels to observe to velopharyngeal closure in details.. Due to good progress of the results the recent data are going to be presented in congress and international peer review journals.

Strategy for Future Research Activity |
(1) Compare articulatory organs in movement using MRI movie, a 3D dynamic images for non-CLP and CLP group, considering the level of articulation disorder in CLP subjects. Next, (2) using fMRI data compare the brain control mechanisms during C and O articulation for both groups. And (3) elucidate the association of articulation disorder levels and brain activation of CLP subjects.

Causes of Carryover |
(1) I am currently working on the use of 3-D MRI images reconstruction using ExpertINTAGE software to compare articulatory organs in movement using MRI movie, a 3D dynamic images for non-CLP and CLP group, considering the level of articulation disorder in CLP subjects. Next, (2) using fMRI data compare the brain control mechanisms during C and O articulation for both groups. And (3) elucidate the association of articulation disorder levels and brain activation of CLP subjects.

Expenditure Plan for Carryover Budget |
I am comparing differences in the oropharyngeal morphology, structures and interaction during motion in CLP and non CLP subjects using multi-view observation, that measurements results require statistical software to analyze. For fMRI, original MRI data will be stored on a large capacity hard disk because of the amount data collected during acquisition. Images of fMRI data will be analyzed using SPM software, implemented in MATLAB. Statistical tests will be performed for the group and for each individual to determine whether changes in regional BOLD signal are significantly related to the task. Presentation software are required to present the data, the results will be introduced in international congress, also published peer review journal using research funds in the next fiscal year.
