本当に重症型グルタル酸血症2型にベザフィブラートは無効か？

Outline of Annual Research Achievements

本研究の目的は重症型グルタル酸血症2型（GA2)患者の皮膚線維芽細胞を用いたベザフィブラート（BEZ)の有効性と、その作用機序の解明である。H26年度の時点で、重症型GA2の中でも合併奇形のない病型ではin vitro probe assayでは、軽症型と同程度にBEZが有効という結果が得られた。H27年度は重症型GA2に対するBEZの効果をアセチルCoAの産生、ATP産生、糖消費率などで多角的に評価したが、明らかな有効性を示す結果は得られなかった。ただし、これはサンプル調整や実験手技の問題が多分にあり、BEZは無効と判断できるような結果ではなかった。また放射性同位元素を用いたβ酸化能分析法を応用してBEZの効果判定を計画し、予備実験まで行ったが、期間内には明らかな実験結果を得る段階まで進められなかった。
以上の研究結果より、新生児期に発症する重症型GA2であっても、出生時に合併奇形を認めない場合にはBEZが有効かもしれないことが分かったが、その作用機序などについては不明のままである。合併奇形をもつ最重症型GA2に対してはBEZは無効である可能性が高いが、大量BEZ投与については更なる検討が必要である。
研究期間を通じた成果として「Efficacy of bezafibrate on fibroblasts of 12 glutaric acidemia type II patients evaluated using an in vitro probe acylcarnitine assay」と題した論文をBrain and Developmentに投稿し（査読あり、謝辞あり）、H28年3月末現在で、riviseとなっている。
