Molecular Genetic Dissection and Neonatal/Infantile Intrahepatic Cholestasis Using Targeted Next-Generation Sequencing

Outline of Annual Research Achievements |
全109例を解析した.28例(26%)で分子遺伝学的診断がついた.内訳はアラジール症候群(ALGS)12例(JAG1 11例,NOTCH2 1例),新生児Dubin-Johnson症候群(DJS)5 例(ABCC2 5例),シトリン欠損症(NICCD) 5例(SLC25A13 5例),進行性家族性肝内胆汁うっ滞症/良性反復性肝内胆汁うっ滞症(PFIC/BRIC) 6例(ATP8B1 2例,ABCB11 4例)であった.サブグループ別にみると,genetic cholestasis群22/31(71%;ALGS 11例,新生児DJS 2例,NICCD 4例,PFIC/BRIC 5例),unknown etiology with complications群2/46(4.3%;ALGS 1例,PFIC/BRIC 1例),unknown etiology without complications群4/32(12.5%;新生児DJS 3例,NICCD 1例)で分子遺伝学的診断がついた. 診断のついた4疾患群間で統計学的検討を行うと,在胎週数,出生体重,発症月齢,D.Bil,AST,ALT,GGTで有意差を認めた.ALGSはAST/ALT,GGTで高値(272/196 IU/L,645 IU/L),新生児DJSは早期発症し(0ヶ月),D.Bil高値(11 mg/dL),AST/ALT低値(25/16 IU/L)であった.NICCDは発症が遅く(4ヶ月),D.Bil低値(2.6 mg/dL)であった.PFIC/BRICはGGT低値(17 IU/L)であった(括弧内中央値).遺伝子診断の重要性を世界に先駆けて報告した。(Togawa T, Sugiura T, et al. J. Pediatr.2016). Genetic cholestasis群において,分子遺伝学的診断がついた22例は全て臨床診断と一致していた.これは本症において臨床診断の重要性を示すが,一方で6例の原因不明患者が分子遺伝学的解析をもとに正確な診断を得ることができた.
