Research Abstract

We have been performed respiratory function test and pulmonary perfusion scintigraphy in patients with lung cancer in order to achieve pulmonary resection safely. Using these tests, postoperative respiratory function after pulmonary resection was predicted precisely. In addition to these tests, maximum oxygen consumption ( Vo_2 max ) was measured during maximal exercise test in preoperative patients in order to know pulmonary circulation reserve and then, its correlation to the respiratory function test studied.
Till the end of 1985,147 patients with lung cancer were evaluated. In these patients, after respiratory function test was done, maximal oxygen consumption was calculated using expiratory gas analyzer. When DLCO was under 20 ml/min/mmHg and Vo_2 max was under, 1,100 ml/min, it was found that cardiopulmonary function reserve decreased significantly and the incidence of complications increased.
We are now studying anaerobic metabolism during the exercise test. Especially, the relationship between the rapid increase in blood lactate and the quantity of exercise is now under the investigation. These studies are extremely important to estimate cardiopulmonary reserve because the number of high risk aged patients, who undergo pulmonary resection, are increasing from now on.
