Research Abstract

Ischemic heart disease which leads to the cause of sudden death, is the most serious disease in the industrial countries. In Japan, the incidence of heart disease has been ranked next to that of malignant tumor since 1985. The number of death due to ischemic heart disease based on arteriosclerosis of coronary arteries in 1987, have increased 5 times more than that in 1950. The selective coronary angiography is the final diagnostic modality for the ischemic heart disease. But this modality is invasive and costly because a special catheter must be directly inserted into coronary arteries using a special angiographic system, and have hospitalization. Therefore new and safer diagnostic method for ischemic heart disease has been required year by year.
Only k-edge energy subtraction angiography with synchrotron radiation can reveal the coronary artery via the intravenous injection of contrast material. And two dimensional image acquisition system which can demonstrates the images as a conventional cine-angiography, is being constructed at Photon Factory in Tsukuba. This system consists of five elements. 1) Sufficient X-ray flux(Insertion device; Wiggler). 2)High speed energy switching(Above and below K-edge). 3)Beam expansion(Asymmetric reflection on Silicon crystal). 4)High speed image acquisition TV system 5)Image processing.
The experiments with this prototype angiographic system showed that coronary artery could be demonstrated dearly in the animal such as dog. The elements of 3) and 4) were nearly finished. But for the human studies, the most important problems of this system is an insufficient X-ray flux and a high speed energy switching. Insertion device must be developed for a sufficient X-ray flux. And new methods of energy switching might be suitable because mechanical energy switching using the vibration of large silicon crystal might have dynamic problem such as strains of crystal by moment. Further studies will be needed especially in 1)and 2) elements.
