| Outline of Annual Research Achievements |
覚醒下手術予定患者のMRI撮影を術前高磁場撮影を行い、脳溝を中心としたobject-based morphometryに基づき、BrainVISA解析ソフトウェアを用いた入念な術前準備を行った。
計画１実績について：電気刺激装置の位置情報の取得を１４名の患者に対して行った。具体的には、MRI 画像をアップデートしたナビゲーションシステム（BrainLAB 社装置）に読み取り可能な、滅菌されたプローベを準備し電気刺激装置に装着した。画像及び位置情報の読み込みはSlicer 等ソフトウェア上で行い、電気刺激装置の位置情報のログを取得した。ログ情報取得の手法については、研究協力者である山田氏と共に数例の試行を行った後に実際の患者に適用し実現した。成果については学会およびProceedingsとして報告を行った。
計画２実績について：覚醒下手術のビデオにおける検査者タスク提示、電気刺激条件、患者の反応を確認し統合させ、覚醒下手術におけるマッピング工程の可視化の作業を行った。検査者が行う検査タスク（物品呼称、動詞生成、数字のカウンティング、漢字・ひらがな音読、計算）、術者が行う脳表面（MRI で作成した3D 構築脳表画像と顕微鏡映像との相違を確認後）上の電気刺激範囲、反応が起こった位置を電流値と共に可視化する。平成２７年度については２４名の患者の覚醒下手術に参加し行程解析のデータ作成を行った。また、過去のマッピング症例を上記ビデオ記録をもとに１５４名の後方視的解析としてまとめ、学会及びProceedingsとして報告を行った。行程解析については研究協力者である南氏と共同して調査を進め、術後症状も含めさらに研究内容を深化させている。

Reason
現在のところ、ログ情報を手術中に取得できるプロセスは確立させたためログ取得を自動化出来るようなソフトの改良を加えている。今後は全ての覚醒下手術において自動的なログ取得が行える手法を確立できるようさらに研究開発を進めている。

| Strategy for Future Research Activity |
計画１に関しては全ての手術においてログ取得を自動化し、簡便かつ確実にデータ取得したものを保存し解析可能とすることである。術中に電気刺激部位の特定と同時に機能部位のデータベース化も同時に進める。
計画２に関して、マッピングで脳機能反応解析を進めながら閾値の設定を行い脳機能毎の詳細な解析を更に進める。
上記計画１と２の結果を基に、当初の計画であった標準脳作成に向けての研究を加速させる。
