認知症における大脳皮質可塑性障害のメカニズムの解明と新たな早期診断法開発への応用

Outline of Annual Research Achievements

アルツハイマー病（AD）ではアミロイドベータ（Aβ）の脳内蓄積によるシナプス可塑性の障害が指摘されている。またAβが認知症発症の10~15年前からすでに蓄積し始めていることがわかっている。本研究では、当院で開発されたヒト大脳皮質に可塑性変化を効率的に誘導できる4連発経頭蓋磁気刺激法（QPS）を用いて、認知症早期診断への応用に関する検討を行った。
右利きの認知症患者および正常認知機能高齢者を対象として、認知機能検査と髄液中総タウ、Aβ40/42測定を行った。左一次運動野へQPSを行い、長期増強（LTP）様効果を検証した。
早期AD4名、軽度認知障害（aMCI）5名、正常認知機能5名の記録が終了した。正常認知機能群と比べて早期AD群とaMCI群において、髄液中総タウ、AD indexの高値を認めた。またAβ比（Aβ40/Aβ42）は早期AD群が最も高く、次いでaMCI群、正常認知機能群の順であった。正常認知機能群ではQPSによりLTP様効果を誘導できたが、早期AD群やaMCI群ではLTP様効果の誘導が障害されていた。誘導されたLTP様効果とAβ-ratio、AD-index、総タウとには有意な負相関を認め、Aβ42とは有意な正相関を認めた。
QPSにより、比較的早期の段階でシナプス可塑性の機能破綻を捉えることができた。認知症早期診断に有用とされる髄液バイオマーカーと有意な相関を示したことから、経頭蓋磁気刺激法を用いた大脳皮質シナプス可塑性の検討が、認知症早期診断のバイオマーカーとして応用できる可能性が示唆された。
