Outline of Annual Research Achievements

1,てんかん、2,亜急性に悪化する脳腫瘍、3,突然機能を失う脳卒中の三病態に応じてリアルタイム機能フィードバックを加え、多種機能画像によるfast & slow機能回復・変容過程を可視化する。脳情報抽出には脳波(EEG), 脳皮質電位(ECoG)、functional MRI(fMRI), およびtractographyを用いる。病態1、2ではECoGによる高周波律動(HGA)を含めた多帯域リアルタイム周波数解析、運動・言語・認知機能読み取りを行い、その結果を患者にフィードバックし変容した脳機能を可視化する。覚醒下手術では、独自作成した高密度電極ECoGのα、β帯域の脱同期とHGAにRGB色を付してcolor-coded周波数ダイナミクスを解析する。Tamuraら(2016)はHGA解析と皮質-皮質誘発電位(CCEP)を併用し、術中機能ネットワークを局在した。本法は“身体性システム”で生まれた局在法であり、さらなる検証を行う。また申請者が特許を得た脳血流計測法で、開頭術中に課題による脳血流変化、ECoG周波数変化、術前fMRIとの比較を行う。病態3では脳梗塞を対象とし、一次感覚-運動野付近運動想起(Motor Imaginary: MI)-EEGのα-β帯域変化から、MI機能を読み取り、アバター操作と電気刺激(FES)の視覚・触覚フィードバックを行う。発症時とリハビリ終了後にアバター視覚フィードバックMI-fMRIで機能変容を可視化する。
