| Outline of Annual Research Achievements |
2019年はてんかん患者の高次脳機能およびそれに関する神経ネットワークの検討を進めた。まず、術前評価として、脳への長期的な侵襲の程度、部位を知るために発症年齢、罹病期間、発作頻度、焦点部位、使用薬剤、合併症などの情報を収集した。次に、詳細な神経心理学的検査により高次脳機能の特徴を測定し、MRIにより病変部位の有無、SPECT/PETによる血流/代謝低下または亢進部位を確認した。また、脳磁図により発作間欠期の活動からてんかん起始部を推定した。
高次脳機能の神経基盤を検討するために、選択的Wada testと皮質電気刺激による高次脳機能マッピングを施行した。選択的Wada testでは、脳血管外科の技術を応用し、ごく細いカテーテルを用いることにより、中大脳動脈の分枝にpropofolを注入し、その領域のみの機能を検討できる。両側M1,M2,P1で行い、言語、視空間認知、記憶について検査を施行した。皮質電気刺激による高次脳機能マッピングでは、我々の考案した言語・視覚認知課題を用いて皮質電気刺激による機能マッピングを行い、各高次脳機能の関連部位を同定した。
以上を行った結果、言語の側性化について、いくつかのパターンに分かれることが明らかとなった。左前頭葉が表出、左側頭頭頂葉が受容に関わる古典的なパターンは認められたものの、それ以外のパターン、すなわち左半球内での言語機能の分布の違い、右半球に一部または全部の言語機能を認める例など多様であることが分かった。脳損傷の生じた時期により言語機能の分布が変化したことを示唆する例も認められた。また、相貌認知についても後頭側頭葉での局在に個人差があることが示された。

てんかん患者に関するデータの収集は予定通り進行しており、学会発表などの成果も得られている。研究分担者、研究協力者との連携も円滑に行えている。

| Strategy for Future Research Activity |
引き続きてんかん患者の詳細なデータの収集を進めるとともに、これまで十分に行えていなかった高次脳機能と神経放射線学的な知見との対応について解析を進める。その結果を学会や論文として公表するために考察も深めていく。
