Research Abstract

Developmental Reflexは発達に伴い出現し消失する反射、および、出現する反射、として、発達月齢依存的な行動の出現を確認することで識別することができる。胎児期からの新生児、乳児のこれらの行動発達を段階的に経た後、不随性から随意性の出現が生じると考えられる。発達論的解析を目指す本課題では、特に、より早期発達段階として不随性行動と定義される原始反射に対して、腹臥位から仰臥位という大きな体位変化の直後ではなく、時系列動態としてタイムラグを経て出現した質的に異なる行動パターンとして、主体の動機性の存在を推察し得る表現ではないか、と考え着目し、端緒的な論文を報告した[OA　autism、2013]。主観的識別は発達診断上において非常に重要であるが、再現性の点で共有が困難なため、定量情報に基づく本・高次相関機能評価方法として、多変量解析法の探索を進めた。その臨床的検証として、埼玉医科大学・小児科・筧紘子助教の協力を得て、後方視的に新生児集中治療室に入院経験がある児の退院後の追跡を行い、複数かつ多様な入院時診断指標との高次な相関情報を統計的に検討したところ、入院時の脳領域のサイズ、生化学的血液内分子と、神経行動学的発達指標の出現の遅れとの相関性について示唆を得ることができ、論文の投稿を行った。その行動指標である寝返りは不随意性から随意性を伴い、抗重力運動を生後の一定の発達段階を経た後に出現すると考えられることから、胎児期新生児期に予防的介入を可能とする診断法開発にアプローチすることができた可能性がある。
発達モデルシミュレーションを目的とする霊長類の社会学習・情動機能獲得研究においては、同齢間社会性獲得に一定の発達齢依存的な高感受性期が存在する可能性を示唆する論文報告を行い、多変量解析により自閉症児と共通な高次な情動的行動の構造化について、論文報告を行った[Sci Rep]。

Reason

随意性・付随性を推定する困難なアプローチの鍵として、多変量解析の統計的な最適化が必須であること、児や霊長類モデルの発達を実践的に記録することができるシステム開発は大変困難であると予想されたが、いずれも順調に進み、中核となる学術基盤を、論文によって国際的に評価されたと考える。

Strategy for Future Research Activity

臨床研究、および、霊長類モデルによる基礎研究が平行して推進できるように、引き続き、同様にバランスをとり進める予定である。一人ひとりの児の発達を詳細に追跡しているため、例数の蓄積には時間を要す。二年間の研究期間にとどまることなく、継続発展させる必要がある。一人ひとりの異なる状態を丁寧に把握し、最適な介入を探索することで、横断的かつ縦断的な研究達成を目指す。
