Outline of Research at the Start

脳梗塞や脊髄損傷では、脳と脊髄とを繋ぐ神経接続の断絶により身体運動麻痺が生じる。また、脳梗塞や脊髄損傷患者はうつ症状を併発し、それが機能回復を妨げる。このように病態脳では神経接続と心の限界が著しく低下する。しかし辛く厳しいリハビリテーションを懸命に乗り越えることで、残存した神経接続が強化され、機能回復が実現される場合もある。これらのことから、神経接続と心に機能回復を可能にする潜在能力が備わっているのではないかと考えた。本班は、脳梗塞・脊髄損傷により神経接続が低下した病態脳の限界を規定する因子の特定と、その突破法の開発を担当する。

Outline of Final Research Achievements

Human functional brain imaging experiments have shown that activity in the ventral midbrain during movement preparation can predict future muscle output. Activation of neuronal activity by the DREADD improved movement time, muscle output, spinal reflexes, and the thermal response in monkeys. Intervention with an artificial neural connection system in patients with chronic spinal cord injury resulted in increased walking speed, increased neural connection strength between the brain and spinal cord, and expanded the area that can induce walking. We found that recovery of walking function is accompanied by reorganization of neural circuits connecting the brain and spinal cord.

Academic Significance and Societal Importance of the Research Achievements

本研究の成果は、脳梗塞や脊髄損傷患者における神経接続と心の限界を理解し、それを突破するための新たな介入法の開発に貢献する。学術的には、神経科学やリハビリテーション医学において、神経接続と心の役割を明らかにする重要な知見を提供する。社会的には、機能回復を妨げる病態脳の限界を克服するための介入法の開発は、脳梗塞や脊髄損傷患者の生活の質を向上させる可能性がある。新たな治療法やリハビリテーションプロトコルが確立されれば、患者の機能回復が促進され、自立生活への復帰や社会参加の向上が期待される。
