1. Meniscus transplantation using pedicle synovial graft, free synovial graft and fibrin clot In sixty-eight rabbits, the medial meniscus was replaced with an allograft meniscus, that had been preserved at-80゚C for-2weeks, using pedicle synovial graft (group B), free synovial graft (group C)and fibrin clot (group D). As a control, an isolated allograft meniscal transplantation was perfomed (group A). Histological examination ahowed the synovial tissue covered the peripheral surface of the transplanted meniscus in group A,the peripheral one half in group D,the vicinity of inner edge in group C,and the entire surface in group B at 4 weeks sfter transplantation. The connective tissue penetrated the meniscal substance with time, in all groups. Chondrocytes, possibly derived by metaplasia from the connective tissue, were regenerated in group B at 8 weeks and 12-week allograft meniscus of other groups. Microangiographic examination revealed vessels along the peripheral surface of the meniscus in both group C and D and extended to the inner rim in group B at 3 weeks. In group A,there was the vessel on the peripheral surface at 4week. This study has demonstrated pedicle synovial graft, free synovial graft and fibrin clot facilitat the regeneration process of the meniscal allograft. 2. Effect of synovectomy on meniscal allograft transplantation The regeneration process of meniscal allograft after synovectmy was investigated in thirty rabbits. acid was injected in the knee joint at 1 week before meniscal transplantation. Examination by light and electron microscopy showed that the parameniscal synovial tissue healed and the synovial tissue covered the peripheral surface of the meniscus at 8 weeks after transplantation. Although the process of regeneration of allografted meniscus, that underwent synovectomy, was essentially the same as an isolated allograft meniscus as a control, synovectomy delayd the response of regeneration.
