(1) Collagenase production was stimulated by nasal secretions from patients with chronic sinusitis in vitro, but not by nasal secretions from nasal allergic patients. A soluble factor was isolated from the nasal secretions of patients with chronic sinusitis by ammonium sulfate precipitation and Sephadex G-75 gel chromatography. Collagenase production by nasal polyp-derived fibroblasts was considerably inhibited by anti-interleukin (IL) -1 antibody.
(2) Furthermore, we investigated the localization of IL-1beta mRNA in the maxillary sinus mucosa of patients with chronic sinusitis by in situ hybridization. In chronically inflamed paranasal sinus mucosa, IL-1beta mRNA was detected in some extravascular polymorphonuclear leukocytes (PMNs) and in a small number of mononuclear leukocytes, but not detected in other tissue cells or in intravascular PMNs.
(3) We investigated the effects of roxithromycin (RXM) on the mRNA expressions of proinflammatory cytokines, IL-1beta, IL-6, IL-8 and tumor necr
osis factor alpha (TNFalpha), of human histiocytic lymphoma cell line U937, and human cultured fibroblasts and epithelial cells derived from nasal mucosa. Without stimulation, these cells expressed a trace amount of cytokine mRNA that could be detected by RT-PCR but not by Northem blot analysis. After stimulation for 4 hours by 1 ng/ml of rIL-1beta, IL-8 mRNA expressions of these cultured cells were partially suppressed by 0.5 to 20.0 mug/ml of roxithromycin (RXM). RXM also suppressed rIL-1beta induced IL-1beta mRNA expressions of U937 cells and fibroblasts and TNFalpha mRNA expressions of U937 cells and epithelial cells.
(4) The YAMIK sinus catheter (YAMIK) with the administration of RMX was used as a new therapeutic modality for the patients with chronic sinusitis in the out-patients clinic. In the 30 cases without nasal polyps, within one month subjective symptoms such as headache, nasal obstruction and postnasal drip were reduced in 23 casea (78%) after the treatment of YAMIK and RXM,and X-ray examination showed the reduced parasinus shadows in 18 cases (60%) including 9 cases (30%) with complete cure. The saccharine time significantly improved (p<0.01) after this treatment, which indicated the recovery of the mucocilliary function of the upper respiratory mucosa. Less
