Research Abstract |
We have examined the use of a costochondral autograft with rib bone in an articular cartilage defect. Japanese white rabbits were used. After 6,12,24,36 and 48 weeks the rabbits were sacrificed. In order to evaluate the bony union and the quality of the cartilage matrix, histological sections of the graft were stained with hematoxylin-eosin, safranin-O,and investigated immunohistologically by using anti-collagen type I and II antibodies. At 48 weeks after transplantation, the erosion was observed on the surface of the costal cartilage grafts without bone, and also the fibrillations were seen in the cartilage matrix. There were more fissures, and some chondrocytes in the deep layrs were dead. By contrast, the surface of the graft with rib bone was smooth. There were little fissures in the matrix and no necrosis of the chondrocytes was seen. There were type II collagen in the costal cartilage close to the rib bone. A thin layr of cartilage of the graft with bone may be advantageous to the chondrocytes, allowing the diffusion of nutrients from joints fluid. The costal cartilage graft with bone yielded greater stability because of the bony union between the graft and the host. It is therefore suggested that costal cartilage graft with rib bone might provide a useful treatment for the osteochondral defects of articular cartilage, such as osteochondritis dissecans, idiopathic osteonecrosis and osteochondral fractures.
