Research Abstract

MEK/ERKシグナルが恒常的に活性化している白血病細胞株HL60、NB4について、既存の抗癌剤(シタラビン、ATRA)とチロシンキナーゼの下流シグナル阻害剤(MEK阻害剤AZD6244)との併用により、単剤に暴露されたときよりも更に効果的に細胞増殖が抑制されることをMTTアッセイやコロニーアッセイで明らかにした。また、併用の際、薬剤の投与の順番を変えることによっても相乗効果の程度に変化が見られ、薬剤投与の順番も重要であることを確認した。またこれらの薬剤を併用することによって、相乗的に細胞増殖抑制やアポトーシス誘導能が高められることを明らかにした。さらに、MEK/ERKシグナルが恒常的に活性化している白血病患者の末梢血から採取した白血病細胞においても、これらの薬剤の併用による相乗的な細胞増殖抑制効果を確認し、このような薬剤の組み合わせが実際の臨床の現場においても効果的ではないかと思われる。また、長期間のチロシンキナーゼ阻害剤投与による薬剤耐性化のメカニズムを検討し、臨床の現場における問題点の克服につなげていきたいと考えた。まず、チロシンキナーゼ阻害剤(sunitinib)を白血病細胞株に長期間投与し、薬剤耐性株を樹立した。そして、この耐性株を用いて耐性化の機序の検討を行った。その結果、親株ではsunitinibによって抑制されるJAK2-STAT5シグナル活性が耐性株では抑制されないことを確認した。また、耐性株では親株に比べてJAK2を活性化させるIL-6と、IL-6のプロモーター領域に結合する核内転写因子c-Junの量が増加していた。そこで耐性株をIL-6阻害剤やJAK2阻害剤で前処理しその後sunitinibを投与してみたところ、薬剤耐性株は親株同様suitinibへの感受性を取り戻した。以上の結果から、sunitinibによってc-Junを介してIL-6が誘導され、このIL-6によってJAK2-STAT5シグナルが活性化、薬剤耐性化を引き起こしたのではないかと考えられる。このようなケースでは、IL-6やJAK2の阻害によって耐性化が克服される可能性が示唆された。このように症例の状態に合わせて適切な薬剤を組み合わせることでより効果的な治療法となりえることを見出した。
