原発性乳癌の新しい診療アルゴリズムに関する研究

Research Abstract

原発性乳癌の予後因子である腋窩リンパ節転移の有無を治療開始前に予測する数理モデルの開発・評価を行った。国内2施設から291例の症例データを得て予測モデルを作成し、韓国ソウル大学病院の174例を用いて評価を行った。予測モデル作成は決定木アルゴリズムの一つであるAlternating Decision Tree(ADTree)を採用した。構築されたモデルは、学習用データでROC曲線下面積(AUC)=0.770、独立した評価用データでAUC=0.772と高い汎用性を示した。モデルによる予測確率20%以下をリンパ節転移低リスクと仮定すると偽陰性率は5.3%であった。また、ADTreeモデルはデータ欠損にも耐性を示した。さらに同一データでの精度比較では、既存モデルはADTreeモデルに及ぼなかった。本結果を国際学会にて報告し、現在論文投稿中である。さらに、Web上で予測確率を計算可能なサイトを作成し、多施設共同で前向き評価試験を遂行中である。
術前化学療法による病理学的完全奏効(Pathological complete response,pCR)を治療開始前に予測する数理モデルの開発・評価を行った。国内のがんセンター・大学病院より得られた150例のデータを用いて予測モデルを作成し、臨床試験から得た173例のデータにより評価を行った。モデル作成にはADTreeアルゴリズムを用いた。
ADTreeモデルは、学習用データでAUC=0.766,評価用データで0.787と高い予測精度を示した。予測確率20%以下をpCRの可能性が低い群と仮定した場合、偽陰性率は7.7%であった。ADTreeモデルはエストロゲン受容体(ER)陽性のluminal type乳癌においてAUC=0.779と高い予測精度を示した。一方、ER陰性HER2陰性のTriple negative乳癌では、AUC=0.531と予測精度改善が必要と考えられた。同一データでの精度比較では、既存モデルはADTreeモデルに及ばなかった。本結果を国際学会にて報告し、現在論文投稿中である。さらに、Web上で予測確率を計算可能なサイトを作成し、多施設共同で前向き評価試験を計画中である。
閉経後ER陽性乳癌患者を対象としてアロマターゼ阻害剤(exemestane)による術前療法を行った臨床試験で、臨床的治療効果の予測因子を探索した。その結果、年齢、腫瘍径、ER発現度、増殖マーカー(Ki-67)と治療効果との関連が示唆された。さらに、Body mass index (BMI)が低値(BMI<22kg/m2)の群は、中間値(22≦BMI<25kg/m2)や高値(BMI≧25kg/m2)群に比較して、奏効率が低い事が示された(21.7%,56.0%,60.6%,p=0.01)。さらに多変量解析でもBMI低値は独立した効果不良の予測因子であった。この結果を論文報告し国際学会で発表した。
