Research Abstract |
The purpose of the present study was to objectively review the pathological condition of chronic pain and evaluate the efficacy of SPECT cerebral blood flow analysis and statistic image analysis in judgment of outcome of SCS treatment. A total of 14 patients with chronic pain underwent Tc-99m-HMPAO SPECT cerebral blood flow analysis before and after SCS treatment. 3D-SSP was adopted to evaluate the reproduced images. The normal controls were 15 individuals who underwent MRI and were told that no organic abnormalities were noted. Nine patients responded favorably to SCS treatment, while five responded poorly to this treatment. oup and that between the posttreatment cerebral blood flow distribution in the ineffective group and the data obtained from the normal control group and (3)comparison between the pretreatment and posttreatment cerebral blood flow distribution in the effective group and that between the pretreatment and post-treatment data in the ineffective group. (1)According to the baseline data, the blood flow in the prefrontal region, anterior cingulate cortex and secondary somatosensory cortex (SII) either decreased or increased. (2)After SCS treatment, the regions showing decreased blood flow were examined and the statistical pixel-by-pixel summation scores (Z scores) of the prefrontal region, SII and anterior cingulate cortex were decreased in the effective group. In the ineffective group, the regions showing decreased blood flow were also examined and Z scores of the prefrontal region, SII and anterior cingulate cortex were increased. (3)The significant changes in regional cerebral blood flow distribution were observed in the effective group, while only slight changes were recognized in the ineffective group.Tc-99m-HMPAO SPECT analysis and 3D-SSPs seemed to be effective in objectively evaluating the severity and improvement of pain in the patients with chronic pain.
