Research Abstract

II型糖尿病に対して適切な血糖コントロールを行なわない場合、難治性腎症を併発し末期腎不全に陥る。現在までのところ腎不全進展の詳細なメカニズムは明かにされておらず決定的な治療法も確立されていないが、近年、尿細管糸球体フィードバック(Tubuloglomerular feedback : TGF)機構の関与が指摘されている。本研究において申請者は、手技的に困難とされていたTGF反応の評価を、in vivoで高感度CCDカメラを使用して腎輸入動脈を可視化して、その血管反応をコンピューター解析することにより可能とした。また、腎間質内物質動態の経時的測定を可能にする腎微小透析法を開発することによりTGF機構の伝達物質である腎細胞外ATPあるいはTGF反応の修飾因子であるアンジオテンシンII・活性酸素・一酸化窒素の腎間質内濃度の経時的測定に成功した。これらの実験により申請者は、1 マクラデンサ細胞から間質中へ分泌された細胞外ATPが選択的に腎輸入細動脈の血管抵抗値を調節することによってTGF反応が制御されていること、2 分泌されたATPより代謝されたアデノシンがアデノシンA_<2A>レセプターを介してTGF反応を修飾していること、3 TGF反応の修飾因子であるアンジオテンシンIIは腎間質中に血漿中の数十倍の高濃度で存在しており、全身のレニン・アンジオテンシン系とは完全に独立して機能していること、4 アンジオテンシンIIにより産生が刺激される活性酸素および一酸化窒素もTGF反応の修飾因子となりうること、を証明した。さらに最近のデータでは、II型糖尿病のモデルラットであるOLETFラットにおいて、TGF機構の異常が糖尿病早期から存在していることを明らかにしている。以上、本研究ではII型糖尿病では腎内局所レニン・アンジオテンシン系およびATPの腎間質内への分泌異常がTGF機構の恒常性を失わせ、腎症が進展させている可能性を強く示唆する実験結果を得た。今後はII型糖尿病性腎症進展の予防および治療法として、TGF機構の機能維持に対するAT_1、受容体拮抗薬あるいは抗酸化剤の有用性を検討したいと考えている。
