Research Abstract

平成15年度までの研究で、脳波デジタル情報解析についてはその有用性を明らかにできた。すなわち、急性期の異常の検出にはデルタ波では0.53-1Hzの成分、シータ波では4-6Hzおよび6-8Hzの成分の解析が有用であった。一方、臍帯血マーカーについてはANP・BNPを測定したが、脳室周囲白質軟化症との関連はみられず有用でないことが明らかになった。したがって、昨年度から準備を行い、新たな4種類の臍帯血マーカー(アンギオテンシン変換酵素・MDA化LDL・P-セレクチン・8-イソプロスタン)について検討を行った。
脳室周囲白質軟化症の患者では出生後早期に一過性の乏尿がみられ、出生前の血流再分配による腎前性腎不全の存在が示唆される。この現象に関与している可能性を考え、アンギオテンシン変換酵素を測定した。P-セレクチンは、様々な機序による脳障害において血管内皮障害マーカーとして有用であることが知られている。脳室周囲白質軟化症には虚血の関与が考えられ、再潅流障害に酸化ストレスが関与している可能性がある。脳室周囲白質軟化症において酸化ストレスが証明できれば、ラジカルスカベンジャーを治療薬として使用できる可能性があるため、MDA化LDL・8-イソプロスタンにより酸化ストレスを検討した。
今年度は脳室周囲白質軟化症の2例を含む20例の在胎33週以下の早産児について、上記の4種類のマーカーを測定することが可能であった。現時点では、どのマーカーにも統計学的有意差は得られておらず、脳室周囲白質軟化症との関連は明らかになっていない。しかし、症例数が少ないため今後症例の蓄積を行う必要があると考える。
