Research Abstract |
Backgrounds : This study was proceeded to evaluate the difference of plasma stressor hormone changes in 3 types of general anesthesia methods and correlation between the stressor hormone and postoperative pain. Methods: Patient undergoing mandibular ostetomy were divided into the following 3 types of anesthesia methods, GOS, TIVA and GOS/Rop (GOS adds local anesthesia with Ropivacaine). Plasma levels of ACTH and catecholamines were determined in control and 1h, 2h, 3h after induction of anesthesia. All patients received fentanyl by intravenous patient-controlled analgesia (PCA) for postoperative pain control while 6h after surgery. The postoperative pain assessment was evaluated to count the times of analgesic request as well as VAS score. Results : Plasma level of ACTH is significantly increased at 2h and 3h after induction of anesthesia in GOS group. Plasma level of epinephrine is significantly increased at 2h after induction of anesthesia in GOS and GOS/Rop group, and at 3h after induction in GOS group. No significant changes were found with plasma norepinephrine and dopamine in all 3 groups. No significant difference was found both PCA requests and VAS score while 3 groups. Plasma level of ACTH is significantly correlate with PCA requests in GOS and TIVA group and with average VAS score in GOS group. Plasma level of epinephrine is significantly correlate with VAS score in GOS and TIVA group.
