| Outline of Final Research Achievements |
This study showed that nocturnal hypertension, which increases cardiovascular risk, was frequently observed in 69% of patients with Parkinson's disease (PD). Although orthostatic hypotension (OH) has been reported as a risk factor for nocturnal hypertension, minimum systolic blood pressure during activity period and nocturia were better predictors of nocturnal hypertension than OH. Sleep-related factors other than nocturia did not necessarily show an association with nocturnal hypertension. Therefore, sleep disorders might be less likely to be directly involved in nocturnal hypertension of PD.

| Academic Significance and Societal Importance of the Research Achievements |
過去の海外の検討では、PDの夜間高血圧（non-dipper型、reverse-dipper型）の頻度は31.6～92.3％と報告されており非常に幅がある。日本人67例を対象とした本研究では、69％の患者に夜間高血圧を認め、PDでは一般的にみられる所見であることが明らかになった。また、夜間高血圧に影響する要因として、夜間頻尿の関与が示唆されたことから、排尿障害への対策が夜間血圧調節の是正に有用かもしれない。
