Research Abstract

To clarify nursing practice skills necessary to establish community care system in remote areas, we investigated community nursing activities of health nurses involved in establishment of community care system in remote areas reality and issues of health nurses involved in establishment of community care system. The results were as following : :
(1)We interviewed nurses and care-takers in remote areas, mountainous areas and heavy snow areas which already have community care system established concerning factors and processes in establishing community care system and needs of residents before and after establishing community care system. It became clear that health nurses, after identifying issues in their daily nursing activities, further reviewed them in field survey and then established community care system through preparatory process, initial process and midterm process, incorporating the opinions of residents and cooperating with other professionals. At the same time, community care system was evaluated and efforts were made constantly to appeal to the residents in order to maintain the system in place.
(2) A nationwide questionnaire survey with 1,000 health nurses was conducted to investigate their practice, difficulties, necessary knowledge and skills in establishing community care system. Effective response rate was 46.7%. More than 90% recognized that it was their role to establish community care system and 60% of respondents were actually involved. However, about 90% felt it was difficult to establish community care system and cited lack of knowledge and skill as reasons.
Such findings point out the need to provide training and support, especially to inexperienced health nurses, on skills for regional assessment and cooperation with local residents rather than individual and group support skill. Especially in remote areas, not only skills and technology but support is considered necessary to establish care system that meets the needs of residents
