Outline of Final Research Achievements

We submitted an article entitled “Continued and new use of common drugs for the incidence of community-acquired acute kidney injury: cohort and case-crossover study” for a peer-reviewed English journal (Currently under review). This study aimed to evaluate the association between the baseline and new drug use and community-acquired acute kidney injury (CA-AKI). It is important to know more about CA-AKI, as it is preventable and has the potential to progress to chronic kidney disease. In this study, we used both a cohort design and a case-crossover design to evaluate the risk associated with both the baseline and new drug use. We identified that baseline use of renin-angiotensin-aldosterone system inhibitors was associated with increased CA-AKI risk and new use of diuretics, anti-infectious drugs, and contrast medium was associated with increased CA-AKI risk.

Academic Significance and Societal Importance of the Research Achievements

院外発症の急性腎障害の発生が世界的に増加しており、その予防のために一般住民における潜在的なリスク因子の同定が重要である。本研究では、日本の予防介入の機会として最も活用すべきである健診結果に、レセプトデータを組み合わせ、プライマリケアの段階で予防可能な薬剤リスクの同定を行った。本研究は、プライマリケアの現場で一般的に処方されるいくつかの薬剤において、継続処方及び新規処方のいずれも急性腎症がの発症に寄与している可能性があることを結論づけている。今後、関するイベントも考慮して検討を重ねることにより、致命率の高い急性腎障害の発症を減らすことができる可能性があり、本研究の学術的意義は大きいと考えている。
