Outline of Annual Research Achievements

正常例、軽度認知機能低下症例、Alzheimer病症例を対象として拡散テンソル画像を撮像を続けている。各症例でMini mental state examination(MMSE)による重篤度の評価を行っているところである。撮像は3.0-T臨床用MRI 装置を用いて行っている。 拡散画像はEPI法を用いて撮像している。上記撮像により得られたデータから、拡散テンソル画像を、dTV.II.13k +ソフトウェア(広島市立大学大学院情報科学研究科)を用いて生成している。 このソフトウェアは、拡散異方性マップおよび拡散率マップを含む拡散テンソルの計算画像を作成し、さらに、x軸、y軸およびz軸方向の拡散率を計算することができる。放線冠部で投射線維領域、連合線維領域、皮質下領域においたROIでそれぞれx,y,z方向の拡散能を測定している。
現時点での結果として、投射線維領域、連合線維領域では、投射線維方向(z方向)、連合線維方向(y方向)の拡散能はMMSEスコアと逆相関して、ADが重篤であるほど拡散能が上昇するのに対して、血管周囲腔の走行方向(x方向)の拡散能は、MMSEスコアと相関して、ADが重篤であるほど拡散能が低下する傾向を示している。
血管周囲腔の走行方向に限局した拡散能がGlymphaticシステムの活性に相関すると仮定して、血管周囲腔の方向の拡散能を、血管周囲腔、および主な白質線維の走行方向と直交する方向の拡散能との比(ALPS index)で評価している。
ALPS indexはMMSEスコアと有意に逆相関し、また年齢と有意に相関した。ALPS indexがGlymphaticシステムの機能を反映した指標となり得る可能性が示唆された。

引き続き症例も蓄積しつつある。
MMSEスコアと投射線維や連合線維に沿った拡散率との間に有意な負の相関があることを示したが、これはADまたはMCIに起因する投射線維や連合線維における白質変性によって説明することができる。ALPS indexがGlymphaticシステムの機能を反映した指標となり得る可能性が示唆された。
これまでの研究結果を、令和元年5月に開催される国際磁気共鳴医学会で発表の予定である。

Strategy for Future Research Activity

引き続き症例の蓄積を行う。
臨床症状との関連を重点的に評価し、アルツハイマー病、その前段階とされる軽度認知障害(mild cognitive impairment :MCI)さらにその前段階といわれる主観的認知障害(subjective cognitive impairment :SCI)にもDTI-ALPSを応用して、アルツハイマー病の発症に関連したグリンパティック系の変化を探り、発症に至るまでの変化を明らかにしたい。
