Outline of Final Research Achievements |
We attempted to improve anal function through adipose-derived stem cell transplantation by using anal insufficiency rats. Although anal function improved with the transplantation of adipose-derived stem cells, we were unable to demonstrate histologically the regeneration of muscle tissue.
Considering the difficulty of improving anal function through adipose-derived stem cell transplantation in humans, we attempted to improve anal function through anal sphincter stimulation. In rats, improvement in anal function was observed due to muscle hypertrophy, but this has not yet been applied to humans so far.

Academic Significance and Societal Importance of the Research Achievements |
我々の本来の研究の目的は、直腸癌患者に対する肛門温存手術後に生じる患者の肛門機能低下を改善させ、患者のQOLを向上させることにあった。肛門温存手術では、自然肛門が温存される反面、頻便や便の漏出などの肛門機能の低下がしばしば生じる。直腸を再生させる研究は困難と考え、肛門括約筋の再生を試みた。脂肪幹細胞は現在、実用するまでに進歩しているが、クローン病の痔瘻手術の再発予防などへの仕様に限られ、肛門機能の改善に関係する横紋筋の再生には至っていない。我々は、残存肛門括約筋を増強させる方向性に転換し、この方法であれば臨床応用も現実味を帯びてきていると感じている。
