血管の恒常性維持とリモデリングにおける外膜周囲脂肪組織の役割

Research Abstract

冠動脈は通常心外膜脂肪組織によって覆われているがその生理的、物理的意義は不明であった。本研究では、動脈硬化進展と破綻に関する新規制御機構を明らかにすることを試みた。血管外膜周囲に豊富に存在する脂肪組織の形質を、皮下脂肪、内臓脂肪と比較した。血管外脂肪組織は通常は皮下脂肪に近いものの、血管傷害を契機に内臓脂肪型に形質を転換することが明らかとなった。特に、アディポネクチンの低下、IL-6,TNFα、PAI-1の増加が目立った。3T3L1細胞をin vitroで進展して場合は、同様の変化は認められなかったことから、血管拡張による外部からの進展刺激がアディポカインの発現変化を誘導するのではないことが明らかとなった。TNFαノックアウトマウスでは炎症性サイトカインの誘導が抑制された。またTNFαの外部からの投与では、炎症性サイトカインの発現が誘導された。以上より、血管傷害により血管壁で誘導されたTNFαが外膜周囲脂肪組織の遺伝子発現変化に重要であることと考えられた。血管外膜周囲脂肪組織から遊離されるアディポサイトカインの血管リモデリングにおける生理的意義を各種遺伝子改変マウスを用いて検討した。内因性の脂肪組織を除去した後、アディポネクチンノックアウトマウスからの皮下脂肪を移植しても、血管外周囲脂肪組織による防御的機能は観察されなかった。野生型マウスからの皮下脂肪では抑制効果が確認できた。以上より、外膜周囲脂肪組織は、通常病的血管リモデリングに対して、防御的に機能することが明らかとなった。また、血管障害や各種メタボリックな病態において、外膜周囲脂肪組織の遺伝子発現が変化することが明らかとなった。
