Research Abstract

副鼻腔呼吸粘膜に好酸球浸潤が高度になると嗅覚障害を呈する割合が高く、かつ治療にて副鼻腔炎が改善し嗅覚障害の改善も認められた例では、好酸球浸潤も低下した。しかし、手術にて採取した副鼻腔粘膜と嗅粘膜の好酸球浸潤の程度を比較すると、嗅粘膜に好酸球浸潤する割合は呼吸粘膜ほど高度ではなかった。一方、嗅粘膜の分泌腺であるボーマン腺の増生が認められ、レクチン染色でもConAやSNAの発現を示し分泌異常を来している可能性が示唆された。これらのことから、好酸球浸潤した副鼻腔炎での嗅覚障害の病因は、直接の好酸球組織障害蛋白による嗅粘膜障害よりも分泌異常による嗅粘膜表面での嗅分子の接着障害によることが考えられた。
