Outline of Annual Research Achievements

1.三次元線量分布に対する患者個別ガンマ解析法の開発
実測線量分布に基づく体内臓器線量誤差を評価した.ArcCHECKで得られた実測データを基に3次元体内線量分布に変換後,治療計画装置で計算された線量分布と比較を行った.全骨盤照射をした症例では,膀胱や直腸などで実測線量が治療計画時と比し線量高値であり,それを可視化することが可能であった.実測線量分布と治療計画時の線量分布の一致度を示すガンマパス率(γ2%/2mm)で評価した結果,95%以上の一致度を示した.生じた線量誤差は許容範囲内であることを定量的に明らかにした.
2.機械学習を用いた簡易的品質保証法の開発
高精度放射線治療を施行した症例の治療計画と線量分布から取得した特徴量を用い,照射精度を示すガンマパス率の予測モデルを開発し,その精度を評価した.
京都大学医学部附属病院放射線治療科で,2015年から2019年までに高精度放射線治療を実施した888例を対象とし,対象群を学習群と検証群に分割した.治療計画の複雑さ,治療機器の種類,エネルギーなどの治療計画情報に基づく特徴量と,治療計画装置で算出した線量分布に基づく特徴量を取得した.取得した特徴量を基に学習群において,(a) 治療計画特徴量のモデル,(b) 線量分布特徴量のモデル,(c) 混合モデルを機械学習で構築した.構築したモデルを検証群に適応させ,ガンマパス率(γ2%/2mm)の予測精度の性能評価を行った.実測値と予測値の誤差は,混合モデルで最も低く,γ2%/2mmで-0.1%+/-5.6% (平均+/-標準偏差)であった.本研究より,線量分布から取得した特徴量が,ガンマパス率の予測精度を向上させることを明らかにした.
