Outline of Final Research Achievements

We tried to identify the involved innate immune pathways using the mRNA expression profiles of innate immunity-related genes in the tonsils of children with PFAPA. Unbiased hierarchical clustering analysis revealed that the patients could be classified into 2 distinct subtypes. One distinct gene cluster was identified as being characteristic of each endotype. Upstream regulator analysis indicated that activation of TLRs, IFN-γ production and decrease in 1,25-dihydroxyvitamin D are the putative immune pathways for the induction of endotype-specific genes. Moreover, several clinical manifestations were significantly different between the endotypes, suggesting the presence of endotype-related phenotypes in PFAPA.

Academic Significance and Societal Importance of the Research Achievements

PFAPAは、多くの症例が扁桃炎を随伴し、扁桃摘出術によって症状が改善することから、扁桃組織が病態形成に深く関連していると考えられる。しかし、本疾患に関する既報は、血液を解析したものが多く、扁桃組織を解析した報告は少ない。また、自然免疫が発熱に関与していることが推測されているが、その詳細は分かっていない。本研究は、PFAPAの扁桃組織における自然免疫関連遺伝子の発現変化と、発熱を惹起する分子経路を解析した研究であり、これまでに報告されていない内容である。
