| Outline of Final Research Achievements |
In patients with anterior cruciate ligament (ACL) injury, quadriceps muscle strength, muscle thickness, and muscle fatigue were measured before surgery, 3 months after surgery, and 9 months after surgery. Muscle strength of the affected quadriceps muscle was significantly decreased 3 months after surgery compared to before surgery. In addition, muscle thickness of the affected side was significantly decreased 3 months after surgery in the vastus medialis longitudinal (VML) and vastus medialis oblique (VMO) compared to before surgery. The median frequency (MF) slope of surface electromyography power spectrum analysis was significantly increased 3 months after surgery. This suggests that type II fibers of the affected side vastus medialis may selectively cause muscle atrophy 3 months after surgery compared to before surgery. However, because there were not enough subjects who completed the measurement at 9 months after surgery, measurements will be continued in the future.

| Academic Significance and Societal Importance of the Research Achievements |
術前からスポーツ復帰に至るまでの大腿四頭筋線維組成を評価した研究はなく、筋線維タイプによって効果的な筋力強化訓練方法は異なるが、各時期に応じた適切かつ合理的な筋力強化訓練は未だ確立されていない。本研究ではACLR後3ヶ月時点でtypeⅡ線維が選択的に筋萎縮を引き起こしている可能性があるため、術後早期に内側広筋に対して可能な限り高強度でのレジスタンストレーニングをおこなうことによってtypeⅡ線維の選択的萎縮を予防し、スポーツ復帰率及びスポーツパフォーマンスの向上と再損傷の減少に寄与する可能性がある。
