Outline of Annual Research Achievements

中国では微小粒子状物質(PM2.5)及びオゾン(O3)等による大気汚染の深刻な状況が持続しており、住民の健康への影響が懸念されている。特に、呼吸器疾患を有する患者は大気汚染の影響を受けやすいことから、その対策は急務である。近年は大気汚染対策として、空気清浄機を使用する家庭が増加しつつあるが、空気清浄機の使用による健康影響改善の効果については十分に明らかにされていない。本研究では、中国の北京市において空気清浄機使用の効果を評価するためのクロスオーバー介入研究を計画した。
北京市内に在住する慢性閉塞性肺疾患(COPD)患者を対象に、大気中のPM2.5が高濃度となる冬季(11～3月)と、O3が高濃度となる夏季(6～9月)の年2回実施することとした。対象者は無作為に2群に分け、A群は真の空気清浄機（フィルタを装着したもの）、B群は偽の空気清浄機（フィルタを外したもの）を各季節に2週間連続して家庭で使用してもらい、その後に1週間のウオッシュアウト期間を設け、次の2週間はA群とB群で空気清浄機を入れ替える。各調査期間中に、家屋内外の大気汚染物質濃度の測定とともに、肺機能検査、血液等のバイオマーカーの検査、呼吸器症状の評価を繰り返して実施し、家屋内外の大気汚染物質濃度と健康指標との関連と、空気清浄器使用の有無による違いを評価する研究計画を確定して、北京大学及び兵庫医科大学の倫理審査で承認を受けた。また、本研究で使用する測定機器と空気清浄機を選定し、北京大学において環境汚染物質の測定、健康影響評価の手順等についての確認を行った。しかし、中国国内で新型コロナウイルス感染防止対策が徹底されており、調査を開始できない状態が続いている。2022年末に中国政府による対策が緩和されたため、調査を開始する可能性について北京大学の共同研究者との協議を行っている。

Reason

北京大学及び兵庫医科大学での倫理審査の承認、中国政府科学技術部による国際共同研究としての承認は既に2019年度に得られているが、その後の新型コロナウイルス感染拡大に伴って中国国内で感染防止対策が継続され、住民を対象としたフィールド研究を実施することができない状態が続いているため、現在は研究を中断している。

Strategy for Future Research Activity

研究を実施する準備は整っており、現在も中国側研究者とは電子メールやオンラインで連絡を取り合っている。2022年末には中国政府による新型コロナウイルス感染対策が緩和されたが、その後に中国国内で感染が急拡大したために現在も研究は中断している。当初の研究計画の見直しは避けられないが、2023年度中には研究を再開し、できるだけ当初の目的が達成できるように努めたい。
