本研究では膝周囲骨切り術(Around knee osteotomy : AKO)術後患者における歩行時の荷重緩衝能力の指標(負のピーク膝関節パワー(W/kg)並びに膝屈曲角度変化量(rad))に影響する膝の筋機能について調査した。結果、荷重応答期の膝屈曲角度変化量と角速度60°/secおよび角速度180°/secの膝伸展(Rate of torque development : RTD)に有意な正の相関を認めた。これらの結果はAKO後の大腿四頭筋の短時間の力産生率が、荷重緩衝能力の維持改善にとって重要であることを示唆している。

| Academic Significance and Societal Importance of the Research Achievements |
本結果は、AKO術後患者が歩行時の荷重緩衝能力を維持し、長期的な関節保護を実現する上で、大腿四頭筋の短時間の力産生能力が重要であることを示唆している。従来の膝関節術後のリハビリテーションでは膝周囲筋の最大筋力の強化に主眼が置かれてきたが、本研究で得られた知見は、最大筋力よりも短時間の力産生能力が関節保護につながる歩容の正常化にとって、より効果的であることを示唆している。これらの知見は長期関節保護を目的とするより効果的なAKO術後リハビリテーションを構築する上で有用である。
