| Outline of Research at the Start |
本研究では、国連ミレニアム開発目標（MDGs）の中のリプロダクティブ・ヘルス関連指標の改善（とくにMDG５）を目指して、異なる医療政策をとったインドネシア、ラオス、バングラデシュの3か国を比較する。1990年から2015年までの間の3か国のアウトカムを、妊産婦死亡率やその他のリプロダクティブ・ヘルス関連数値、女性のポジティブな出産経験を中心とする出産の質、妊娠・出産時の医療介入の多寡の3点から検討する。それによって、低・中所得国における効果的なマタニティ政策を検討することが可能になる。この成果は、現在進行中のSDGsの実現を促進する上でも重要な示唆を与えるものと言えよう。

| Outline of Final Research Achievements |
A study focusing on maternal and child health in villages in Rajoir District, Madaripur District, Bangladesh, was conducted. Given the close relationship between women's health and their overall lives, the study also investigated the situation of child marriage, the use of microcredit, and women's mobility. As a result of the promotion of facility-based deliveries by MDGs, the facility-based delivery rate in the surveyed village is 67%, and the cesarean section rate is 60% (2021). Consequently, the rate of breastfeeding among children has decreased, and there is an increase in postpartum health problems among women, leading to undesirable conditions for women's health. This is partly attributed to an increase in remittances from overseas labor migration, which has enriched the village, and the proliferation of private hospitals, leading to an increase in medically unnecessary cesarean sections.

| Academic Significance and Societal Importance of the Research Achievements |
MDGsと後続のSDGsは人類に共通の目標を掲げているが、それが個々の文化でどのような形で現れるかについては、文化人類学的な調査が必要である。グローバルヘルスの分野で一般的に主張されていることが、ある文化のなかで生きる人々に与える影響については、その文化の視点から見た調査が必要である。施設分娩が出産の安全性に寄与し、女性の健康の改善に寄与するためには、施設分娩の質が高められなくてはならない。また、政府による私立病院の規制や、女性の側の教育レベルの向上、NGOのヘルスワーカーによる支援も重要である。学術的意義としては、グローバルヘルスを個々の文化に視点を置いて見る重要性を明らかにしたことである。
