地域社会の人間関係の希薄化・縮小化により育児や介護、災害や転居、独居や死別など、いつでもどこでも誰にでも社会的排除のリスクが存在する。社会的包摂の推進は国策の喫緊の課題である。住民のよりよく生きる権利を守るために社会の包容力を志向することは保健師の責務である。現代社会における弱者を包容する地域づくりに取り組む保健師の実践知から社会的包摂を志向する公衆衛生看護の概念構造（原理）を解明し、原理に基づくロジック・モデルを開発する。先ず社会的包摂を志向した保健師の活動の好事例を発掘し、それらの概念構造（原理）を検討し、領域別にロジック・モデルを考案する。最終的にはモデルの実践への適応可能性を担保する。

| Outline of Final Research Achievements |
It is the responsibility of public health nurses to be oriented toward social inclusion, i.e., the ability to include society in order to protect the rights of community residents to live better.
In this study, we elucidated the conceptual structure of social inclusion-oriented public health nursing based on the practical knowledge of public health nurses working to create communities that are inclusive of the vulnerable in modern society, and developed the "Logic Model of Social Inclusion-Oriented Public Health Nursing” as an activity principle that can be applied to practice. This model can be used as a theory of activity principles for public health nurses to develop various activities (district activities, individual support based on district/regional assessment, and proposal, planning, implementation, evaluation, and explanation of various projects) with an awareness of social inclusion in order to protect residents' basic human rights and the well-being of communities.

| Academic Significance and Societal Importance of the Research Achievements |
本研究で開発したロジック・モデルを用いることで、保健師が自身の実践について社会的包摂を志向する活動として捉え直し、地域づくりとして評価することが可能となる。モデル中に示された原則や関わり方の基本についてセルフチェックすることで、社会的包摂を志向しているか自己点検でき、保健師の責任の持ち方に関する意識変容を促進する。
モデルの普及・定着により、保健師が早期に社会的排除の要因の存在に気づき、その状態を予防・改善する取り組みを企画・推進できる。保健師間でこのモデルを共有し、活動原理としてモデルを実践現場に浸透させることにより、保健師が地域づくりを通じて社会的責任を果たす機能を高めることも期待できる。
